60 years male for regular hemodialysis

Patient came for regular hemodialysis. HOW WAS HE DIAGNOSED? 1 year ago, on 18th December,2022 patient had complaints of high grade fever and body pains and went to some hospital in Hanunakonda. All routine investigations were done there(reports are not available) and was diagnosed with Chronic kindney disease. He was advised for regular hemodialysis. Dialysis was being done. PRESENT HISTORY 15 days ago, Patient was feeling weak and unresponsive. His family members took him to nearby local hospital. They were said that he had low sugar level. And he was treated with IV fluids ( ? Unknown) 2 months ago, C/O SOB, insidious in onset and gradually progressive and decreased urine output. He was advised for dialysis and dialysis was done. Since then he is on regular dialysis. PAST HISTORY He is a k/c/o HTN since 2 years and DM since 3 years and on regular medication. 65 years old female with SOB and altered sensorium Dr.Pavan Dr.Navya Dr.Stimita CHIEF COMPLAINTS Patient came to casualty around 7:40 PM with C/O SOB(difficulty in breathing) since 10 days, Fever since 10 days and altered sensorium since 2 days. HISTORY OF PRESENT ILLNESS Patient was apparently asymptomatic 10 days ago, then she developed   SOB which was grade 1 initially, then gradually progressed to grade 4 Fever since 10 days, low grade, gradually progressive Orthopnea+ Altered sensorium since 10 days. FROM A GENERAL MEDICINE INTERN   5 DAYS OF WARD DUTY   Have taken venous samples Have inserted IV cannula Setting up of IV drip Learnt Thorough examination of CNS,CVS,RESPIRATORY SYSTEM,ABDOMEN from PGs(Dr. Lohith Varna,Dr. Prachetan) And tried to explain all examinations (whatever I have learnt) to the UGs. Vitals monitoring      CNS EXAMINATION ON PATIENT TEACHING AND LEARNING  SHOWING UGS HOW TO CHECK VITALS QUICKLY  5 DAYS OF NEPHROLOGY DUTY I was posted in CKD ward, dialysis ward and casualty as well. In dialysis ward - Saw plenty of CKD  patients who come for dialysis 2-3 times every week.
