37 yrs female
House wife

C/c anxiousness to conceive

*Menstrual history*

*Menarchae at 13 yrs
*Regular cycle (very few times get delayed for 2-3 days)
*She bleeds for 4-5 days
*Amount of bleeding is normal
*she uses 2 pads per day
*She gets clots
*Back pain and lower abdominal pain for 1-2 days

*Marital history*

*Married since 17 yrs
*Active sexual life.

*Sexual history*

*1st one year coitus during safe period.Then normal active sexual life.
*not a/w pain after intercourse
*no h/o erectile dysfunction/ejaculatory dysfunction
*coitus is timed during ovulation 

*Past history*

*No DM,HTN,cardiac,renal,CNS disease.
*No h/o TB,asthma
*She never took OCPs
*She is on thyroid medication since 1 year
*UTI(?) (1yr and 1-1.5 months back)for that she took antibiotics from local doctor.
*She gets white discharge since 3 yrs if she stays in empty stomach in the morning

*Allergy history*

*Allergic to egg,brinjal
*She develops rashes over genital area after eating those foods
*Rashes are itchy,painless.

*Family history*

Her mother conceived after 5 years of marriage.

**Partner history**

*41 yrs.
*He had grocery store previously.Now he drives auto.
*He got his all lab investigations done since all these years.He was never given any medication.
But 1 yr back again he got some lab test done,he was given some medication.

Infertility treatment history 
Update on 19/12/21
She was referred to obg dpt.
Thorough physical examination was done.
 lab investigations to get done :  MRI pelvis (patient)
  And semen analysis (her husband)

A 75 years old Male farmer presented to the medicine opd with the   Chief  complaints  of  Swelling of both the feet ,ankles and hands and facial puffiness since 10 days. He also complained of back pain since 2 years and swelling in the left inguinal region since 1 year. Patient being a farmer used to go to his field regularly and was living his normal life.he used to eat regularly and was able perform all his daily activities on his own.Then 2 years back he developed back pain but he neglected.
